New Patient Forms and Information
We are pleased to have the opportunity to assist you with your physical therapy care. Attached, please find our new patient paperwork. If you could please fill it out and bring it with you to your appointment, we would greatly appreciate it.
We do ask that you arrive 10 minutes early for your initial visit for us to process your paperwork. If you forget your paperwork or do not have time to fill it out, please arrive 30 minutes early to complete it at the office. There are a total of 5 forms to be completed.
You do have the option to scan, complete and email the completed form to aide@corepelvicpt.com.
In addition to the paperwork, please bring your picture ID, insurance card(s), a form of payment (if applicable), and a medication list (if applicable) including the dosage and frequency.
Make a list of questions that you have to discuss on your first visit.
List signs and symptoms that can be of importance to the therapist.
Have in writing key past medical history, previous treatments, current medications, and other important information.
Please call our office to verify your insurance benefits.
Arrive 15 minutes early for your first appointment.
Bring with you a copy of your insurance card and a photo ID.
